Dermal fillers have become one of the most popular beauty treatments on the market—favored by celebrities and everyday people alike. Marketed as a quick, safe, and reversible solution to aging, these injectables promise to smooth wrinkles, plump lips, and contour cheeks with minimal downtime.

But according to a Beverly Hills reconstructive surgeon, that youthful glow may come with a long-term cost.

“You Cannot Fill Your Face to Youth”

In a now-viral TikTok video that has racked up over 12.4 million views, Dr. Kami Parsa, a renowned oculoplastic and reconstructive surgeon, offers a sobering reality check about dermal fillers.

“You cannot fill your face to youth,” he says, warning that repeated filler use can lead to irreversible damage and the dreaded “pillow face”—a puffed-up, unnatural look caused by overfilled facial tissue.

If you thought dermal fillers dissolved within a few months, you’re not alone. But as Dr. Parsa reveals, that common beauty belief may be more myth than fact.


MRI Scans Reveal Startling Truth Beneath the Skin

In his video, Dr. Parsa shares MRI scans of a patient who received more than 12 syringes of hyaluronic acid (HA) filler over a six-year period. The scans—highlighted in vivid green—show large clumps of filler still present years after the injections.

Even more alarming? The volume of filler measured in the tissue was nearly 28 cubic centimeters—more than double what was initially injected.

Why? According to Parsa, hyaluronic acid is hydrophilic, meaning it attracts and holds water. Over time, this leads to tissue expansion, swelling, and filler buildup that can distort facial proportions.

“We felt it’s important to share our findings. Less is more when it comes to HA filler,” Parsa told Newsweek.


Filler Buildup Can Last Years

Though marketed as temporary—lasting anywhere from 6 months to 2 years—Parsa says residual filler can remain in the skin for up to 10 years, especially when injections are repeated without proper spacing or correction.

This prolonged presence, combined with the filler’s ability to retain fluid, may result in overfilled syndrome or what many now call “pillow face.”


What Is Pillow Face?

“Pillow face” refers to the unnatural, bloated appearance that develops when fillers are overused or layered without dissolving older material. It happens gradually—session by session—until the face begins to lose its natural definition.

According to Parsa, pillow face typically develops due to:

  • Layering fillers: Multiple sessions before previous fillers dissolve can lead to buildup.

  • Overcorrection: Injecting more volume than necessary in one or more areas.

  • Skin stretching: The weight of excess filler causes facial skin to stretch and sag.

“Every day we see beautiful patients from all around the world who are very overfilled,” the surgeon writes. “I feel like a broken record—but you cannot fill your face to youth.”

He even shared a patient photo showing a woman who needed several corrective procedures to reduce chronic facial swelling caused by filler overload.


“Injecting Poison Into Your Face”?

Dr. Parsa’s revelations struck a nerve on TikTok, with thousands of comments flooding in:

“When it first came out, all injectors said it would only last six months.”
“I got lip filler once and I’m convinced they’ll never go away.”
“Finally, someone is talking about this. There was no warning!”

One commenter summed up what many are now feeling:

“I will never get filler or Botox. It just feels so scary and weird to inject poison into your face.”
